1. Police Certificate Application Form: 1 copy (available at the Embassy)
2. Valid Passport: Original and a photocopy
3. Fingerprint card ※Fingerprints will be taken upon visiting the Embassy.
4. Valid Maltese ID (original to be presented at the counter)
5. Legal basis documenting the necessity of the certificate (copy acceptable)
6. Supporting documents evidencing the necessity of the certificate (copy acceptable)
7. Apostille Application Form (optional) (available at the Embassy)
8. Official Seal Verification Application Form (optional) (available at the Embassy)

【Important Notes】
- Items 5 and 6 are not required when applying for permanent residency in Malta. For all other reasons requiring a police certificate, please consult with us in advance.
- If items 5 and 6 are in a language other than English, a Japanese translation of each is required.
- An Apostille or official seal may be required for the police certificate. Please confirm this requirement with the submitting authority before applying.
- As the certificate is issued by the National Police Agency via the Japanese Ministry of Foreign Affairs, processing time is approximately 2-3 months. Please apply well in advance.
- After application, you may be requested to provide additional fingerprints or supplementary documents.
- Should you require proxy collection, a change of collection location, or urgent processing, please consult with us in advance.
